Requirements For Submission Of Completed Assessment Examination Forms

Please, find below the updated requirements for submission of completed assessment examination forms:

  1. Bank certified cheque payable to the MEDICAL AND DENTAL COUNCIL OF NIGERIA.
  2. Two (2) recent passport size photographs.
  3. Photocopies of all academic qualifications.
  4. Birth certificate or declaration of age certificate.
  5. Evidence of recognition of the institution (Where the applicant completed his/her University education) by the government of the host county.
